I ACCEPT that I have been fully informed about the issues included in this text regarding my rights as a data owner within the scope of Article 11 of the Law and that my personal data and special personal data will be processed for the specified legitimate purposes and transferred to the specified persons, that I have been informed about this matter and that I have read and understood it clearly, without any pressure, threats, errors or fraud.*
